Abstract

Giant primary liver abscess is a rare entity. The diagnosis is often delayed because usually not suspected. Fever and abdominal pain are the most common clinical manifestations. Leukocytosis, high CRP and anemia are the most common laboratory abnormalities. Abdominal CT is the imaging test of choice for diagnosis. Percutaneous drainage and specific antibiotic treatment is sufficient and safe in most patients. Surgical drainage is indicated in cases of failure of conservative treatment. We present the case in a 72-year-old male patient, who required surgical treatment after failure of percutaneous drainage. We discuss aspects related to the etiology, cynical picture, diagnosis and current therapeutic approach.
